What is Certificate of Origin?
Certificate of Origin (CO) is a document that certifies the country of origin of a particular product. The CO is intended solely to prove the origin of goods in order to satisfy customs or trade requirements such as supporting letter of credit. They can also be used as supporting documents for the issuance of corresponding COs by another authorised Chamber. |
- FMM is authorised by the Ministry of International Trade and Industry to issue and endorse Certificate of Origin since January 1982
- FMM is a direct member of the International Chamber of Commerce & Industry (ICC), Paris since 1988
Types of Endorsement
Malaysian Manufactured and Processed Products
For Members and Non-Members |
Application must be accompanied by a copy of each of the following documents which shall be retained by FMM: -
- Invoice
- Packing list
- Bill of Lading
- Other documents that FMM may deem necessary
FMM shall have the right to inspect the premises where manufacturers assemble the goods under application and to check the documents and records of the applicants.
Goods for Re-export
For Members and Non-Members
Application must be accompanied by a copy of each of the following documents which shall be retained by FMM: - |
- Certificate of Origin from the country of export
- Invoice by the manufacturer or the exporter of the country of origin
- Bill of Lading issued to the manufacturer exporter of the country of origin
- Customs Import Declaration Form duly certified by the Malaysian Royal Customs and Excise Department at time of import
- Commercial Invoice of the re-exporter in Malaysia issued to the overseas importer
- Other documents that FMM may deem necessary
FMM shall have the right to inspect the goods under application and to check the documents and records of the Malaysian shippers.

Direct Shipment from a Second Country to a Third Country
For Members and Non-Members
Application must be submitted together with a copy of each of the following documents which shall be retained by FMM: -
- Certificate of Origin from the country of export
- Invoice by the exporter of the country of origin
- Bill of Lading issued to the manufacturer/exporter of the country of origin
- Invoice of the Malaysian shipper to the overseas importer
- Other documents that FMM may deem necessary

Certification of Commercial Invoice and other Shipping Documents
For Members and Non-Members
Documents to be certified may be submitted with the endorsement for any category of Certificates of Origin. FMM shall retain a copy of each of the document(s) so certified.
How to Apply for CO?
FMM Members:
- Submit continuing Letter of Indemnity under company letterhead
- Submit specimen signatories of authorized signatories
Non-FMM Members:
- Submit continuing Letter of Indemnity under company letterhead
- Submit specimen signatories of authorized signatories under company letterhead
- Manufacturing licence
- Complete registration form for basic information on company

- FMM’s network of nine offices located throughout Malaysia offer professional guidance and service on issuance and endorsement of CO.
- It takes only 15 minutes to endorse a CO
Certificate of Origin Forms
The FMM Certificate of Origin Forms are available in pads of 100 copies each from any of FMM offices, addresses of which are listed in this pamphlet.
Applicant shall furnish all particulars, typewritten as required in the certificate.
Provision must be made for an additional copy for retention by FMM. |
